Secure, fast and free access to your medical records from anywhere. Our patient portal was created with you in mind. By agreeing to participate and providing your email address, you will have access to important information about your recent hospital visit, including:
- Procedures performed during your visit
- List of current and past medical issues
- Discharge instructions
- Lists of current medications and medication history
- Radiology and laboratory test results
- And more!

Patients can provide their email address at the time of hospital registration and will automatically receive an email invitation to sign up for the patient portal in 24 hours.
You may also contact portal support for an invite using the support email address listed below. Click the link below to sign in after you have successfully enrolled with the InteliChart Patient Portal.
After you have successfully enrolled with InteliChart, you may also access your health information via the InteliChart Patient Portal App—available on the Apple App Store or Google Play Store. After downloading the app, sign in with your credentials.
